Lead Licensed Vocational Nurse (LVN) – Care Transitions / Recuperative Care

Full-Time | Tuesday–Saturday | 11:00am–7:00pm
Sacramento, CA
$44.18 per hour

A highly respected community-based healthcare organization is seeking a Lead Licensed Vocational Nurse (LVN) to support medication management and care coordination for vulnerable populations in a Recuperative Care setting.

About the Role

As the Lead LVN , you will play a critical role in ensuring safe, compliant, and effective medication management. Working closely with an RN Clinical Supervisor and interdisciplinary teams, you will support patient self-administration of medications, train staff, conduct audits, and help refine workflows that minimize risk and improve outcomes.

 

✨ What You’ll Do

  • Lead and train staff on medication handling, logging, reconciliation, refills, and discharge
  • Monitor compliance with medication management policies and procedures
  • Conduct medication audits and report findings to leadership
  • Support patient intake, assessments, vitals, care coordination, and discharge
  • Educate patients on medications, health promotion, and self-care
  • Coordinate with pharmacies and providers regarding patient medications
  • Complete incident and event reports related to medication errors or workflow issues
  • Maintain accurate documentation in the electronic health record (EHR)
  • Participate in an on-call rotation for urgent after-hours matters
  • Support patients experiencing substance use or mental health crises

 

✅ What We’re Looking For

  • Active California LVN license
  • 2+ years of clinical experience , including medication administration and patient self-administration
  • Strong knowledge of medications, interactions, and substance use effects
  • Experience training staff and documenting competency
  • Ability to remain calm and effective in high-stress or crisis situations
  • Excellent communication, documentation, and customer service skills
  • BLS certification (or willingness to obtain within 90 days)
  • Computer proficiency, including EHR systems
  • Commitment to serving underserved and diverse populations
  • Bilingual skills a plus

 

Schedule & Status

  • Full-Time, Regular (Non-Exempt)
  • Tuesday–Saturday | 11:00am–7:00pm
  • Flexibility required for holidays, weekends, and alternate shifts as needed
